If you would like to make a request to amend your medical records, please complete the Request for Amendment of Health Information Form and return it to the Health Information Management department at National Jewish Health.
Download, complete, and print the Request for Amendment of Health Information Form, making sure to include your handwritten signature.

Medical Record Destruction Notices
On June 24, 2024, National Jewish Medical and Research Center, 1400 Jackson St, Denver, Colorado, 80206, will destroy all medical records of persons now at least 28 years old who received treatment prior to December 31, 2008. Inquiries concerning records should be directed to the Health Information Management Department 303.398.1580 between 8:00am and 4:30pm, Monday-Friday. Inquiries for the entire record will not be accepted after 06/21/2024.
