Patient Forms
If you have been scheduled for a sleep consult, please fill out the Sleep Clinic Questionnaire and return to us prior to your appointment. Please bring copies of all previous sleep-related medical records to your appointment. We will need any prior records in order to provide any new testing or care options.
Please complete and submit the applicable questionnaire.
If required by insurance, please have a referral and/or insurance authorization sent to the National Jewish Health Sleep Center prior to your appointment.

Refer a Patient
To refer a patient, complete our referral form and fax it to us at 303.270.2109. Our staff will take it from there. For your convenience, we can pre-authorize studies and clinic visits for most insurance plans. Our schedulers will call your patient to schedule his/her sleep study or clinic appointment and will notify you of the scheduled date. If you would like pre-completed referral forms, call us at 303.270.2708.
You should receive a faxed report of the results of your patient's sleep study or clinic evaluation within 7-10 business days.
If you would like to schedule a Lunch and Learn for your office, call us at 303.270.2708. Our doctors are also available to speak to healthcare professional groups about sleep disorders.
If a patient needs to be seen for insomnia by Jack Edinger, PhD, who specializes in cognitive behavioral therapy, a referral with a diagnosis of insomnia from a patient’s physician must be sent to the National Jewish Health Sleep Center, regardless of insurance. Accreditation
The main Sleep Center campus is accredited by the American Academy of Sleep Medicine. In order to maintain the AASM accreditation, we must continually demonstrate performance that meets or exceeds the highest level of performance standards. These performance standards are adhered to at all locations.
In addition, National Jewish Health is accredited by the Joint Commission on Accreditation of Healthcare Organizations. This accreditation requires the adherence to strict quality and performance standards.
